Follow a fictional cat’s invoice through the calculation

Suppose a cat receives a fictional $900 invoice: $700 of diagnostic and treatment charges assumed eligible, a $100 examination assumed outside the selected benefits, and $100 routine care assumed excluded. The eligible base is $700, not $900. In an invented percentage-first design, 80% produces $560; subtracting $150 remaining deductible leaves $410, if the remaining limit permits it.

The owner retains $490 of the bill, plus premiums. If a different contract subtracts that deductible before the percentage, payment would instead be $440. These are teaching assumptions, not a prediction about a real cat. The eligibility of each invoice line must be settled before either formula can be used.

The claim has a timeline as well as a total

Arrange the start date, any relevant waiting-period endpoint, first recorded signs, visit date and submission date in sequence. Do not use the date a diagnosis was named as a replacement for every earlier observation. Preserve accurate records even when they complicate the answer. An insurer’s request for information and the veterinarian’s care recommendation serve different roles.
